Skylights are always suspicious... And I suspect the foamboard to be a possible problem also.

I would suggest (and recommend) an inside/outside treatment with Termidor/Phantom. You can ask your present exterminator to make that application, or you can cancel your "contract" and get someone else to do it. Don't take any crap about canceling, just tell them you have "sensitivity" problems with their chemical applications and you need to cancel.

I am surprised that they didn't recommend that treatment to start with.
